Crestline Exempted Village Schools, Crestline St. Joseph Catholic School, and Galion St. Joseph Catholic School contracts for school nursing, which were approved at the May 14, 2014, meeting, need to be signed. Galion City Schools contract is still being considered by the Board of Education in order to clarify retirement benefit requirements under State Teachers Retirement System (STRS).
ACTION ITEM - Dr. Steve Novack, DO, submitted a letter of resignation at the June 10, 2014, meeting, with an effective date of July 9, 2014.
ACTION ITEM - The proposed contract will extend service through December 31, 2014, and allow payment by purchase order rather than personal credit card. Paying the remaining 2014 subscription fees in lump sum also qualifies the Board for a 10% discount. Total fees due for the subscription: $220.50.
ACTION ITEM - Monthly bills for May and June 2014
Daily operations of the department will be difficult without a Health Commissioner, or someone authorized by the Board to carry on administrative tasks (approve purchase requisitions, payroll, food service operation/retail food establishment licenses, etc.) The Board is asked to consider appointing an Interim Health Commissioner. Law Director Thomas Palmer is reviewing a draft resolution for that purpose, should the Board wish to make an appointment. The draft resolution will be attached when it has been approved.
ACTION ITEM - The ODH Billable Vaccine Pilot Project has come to an end and local health departments (LHDs) must now puchase vaccines privately for children with private insurace coverage. This buying group was recommended by the Ohio Department of Health Vaccine Unit, to purchase all vaccines at reduced cost from suppliers. The buying group is facilitated by the Minnesota Department of Health. Avita Pharmacy currently purchases through multiple suppliers; contracting with a single buying group simplifies the purchase of vaccines.
ACTION ITEM - License holder Mary Turner has a history of failure to maintain Big Plate Diner in compliance with the provisions of the Ohio Uniform Food Safety Code. Ms. Turner participated in an Administrative Hearing with the Health Commissioner and Environmental Health Director on March 14, 2014. From that and subsequent meetings came a written Compliance Action Plan to address critical control points, agreed to and signed by Ms. Turner and Ms. Zmuda on March 26, 2014. Standard and critical control point inspections were conducted on June 18, 2014, in response to a complaint. Six critical violations were documented, all of which had been previously addressed in written inspection reports, the Administrative Hearing, and the Compliance Action Plan.
The next regular meeting will be held on Tuesday, August 12, 2014, at 7:30 am at the Galion City Health Department office.
