    ACTION ITEM - Board approval is requested for a memorandum of understanding with Avita Health System, the fiscal agent for Crawford County Health Partners, of which we are a member, for the purposes of conducting a countywide community health assessment (CHA) and creating the subsequent community health improvement plan (CHIP).
